What actually goes bad inside the R&P to create this symptom?

I've never had one apart.

I had an '86 Cutlass that did that, only it would pull hard in one direction; the guy at the parts store called it GM morning sickness.  I changed the rack and that fixed that problem.

It's usually a swelled O ring, but not really worth trying to fix. For the reason Tom mentioned, liability, repair parts are expensive, and you need some special tools to rebuild one.
Welcome to hell. Here's your accordion.
