My guess: Hidden from view behind the truck is a come-along from the passenger door to an eye bolt that's anchor mollied to the block wall.

Seems like I saw that somewhere and the caption said something about roadside mechanics in Cuba. I don't think he is quite as smart as the Cuban that turned the truck into a boat, but then again, we didn't get to see the "build" pics on that project...
